Insurance Billing:

• We require a photo ID for all patients and will scan your insurance card at each visit. 
• Our doctors participate in most major insurance plans.  Because insurance benefit plans are regularly updated, it's a good idea to check with your insurance company before each visit to make sure our doctors are in your current network.
• Insurance company contracts require us to collect your copayment at the time of service.
